Website Development

For any amendments or updates to the website, staff must click on the above icon link and complete the form. This form is not for technical issues.


Design Work

Please complete the above design request form by clicking on the icon link. This can be for anything from leaflets to posters to business cards!


Training and Development Request Form

Please select this form to start a new CPD Training request. You will need to fill out details relating to the training you are requesting.


New Starter Notification

If you are a hiring line manager, please fill out this form to start your new starter process.


Leaver Request Form

If you are a manager, please fill out this form for staff leavers with as much notice as possible so the YGAM support staff can make the necessary arrangements.


Equipment, Furniture and Peripherals Request

Please complete this form to request additional equipment, office furniture or IT peripherals.



Merchandise Request Form

Please fill this form out if you would like merchandise sending ahead of you for a F2F workshop, or other event. Please give AT LEAST TWO WEEKS NOTICE
